ACCIDENT, INCIDENT INVESTIGATION AND ROOT CAUSE ANALYSIS

In this course, we will introduce you to the philosophy, processes, and procedures required to support a marine casualty investigation.

You will learn about safety issues, risk management, the history of ferry and ship losses, and different forms of investigations. And a closer look at relevant conventions about what to prepare during an investigation. About neutral justice and involved organizations. How to approach media. Practical information regarding conducting interviews, including different kinds of witnesses and situations.

We will also look into what can be the cause of an accident – often a complex chain of events and how to handle evidence, both when collecting them and when it is time to analyse them.

Lastly, we wrap up what to have in mind while writing reports and procedures for the finished product. The course was produced in accordance with IMO Assembly Resolution A.849(20) and the Code for the Investigation of Marine Casualties and Incidents. The Incident Investigation Course is based on the IMO Model Course 3.11.
